They have large consequences for the involved elderly and for the costs of care. That is why it is important to prevent falls, preferably by interventions in primary care in which the numbers of vulnerable elderly increase sharply because more of them stay living at home longer instead of being institutionalized. Potentially effective interventions are available but implementation of these interventions in primary care is lagging behind. In this project (physiotherapeutical) interventions to prevent falls are implemented as part of a disease management program for vulnerable elderly of a GP care group and a home care organization, in collaboration with a regional organization of physiotherapists. Collaboration takes place with the regionally dominant health insurer and the municipality. To measure the efficacy of this implementation strategy, the number of patients screened, referred and enrolled in a fall prevention program as a percentage of the registered vulnerable elderly within that practice (implementation index), is compared to care as usual provided by a reference group of general practitioners (from the sentinel stations of NIVEL primary care database) who do not participate in the disease management program for vulnerable elderly. The effect of the fall prevention programs on patient outcome measurements will be determined in terms of balance, mobility, number of falls in the last 12 months, fear for falling, self-efficacy and quality of life.\n\nKeywords: [{"boost": 1, "value": "Valpreventie"}, {"boost": 1, "value": "Ouderen"}, {"boost": 1, "value": "Kwetsbare ouderen"}, {"boost": 1, "value": "Eerstelijnszorg"}, {"boost": 1, "value": "Preventie"}]
