Limitations of MR methods for the study of muscle metabolism in elderly individuals and patients with muscle weakness

Description: The aim of this project is to determine the sensitivity and limits of dynamic 31P MRS in the examination of subjects with low muscle strength at 2 different clinical MR sites (IKEM in Prague and HFMRC in Vienna) with different equipment and to compare and evaluate the results. It is necessary to determine the appropriate parameters of the examination protocol (workload, pedal resistance, magnitude and frequency of pedal depression, MR sequence parameters, etc.) for successful examination of patients with muscle weakness. The possibilities of examination in different magnetic field (3T IKEM, 7T HFMRC) using different types of ergometers (mechanical in IKEM, pneumatic in HFMRC), different coils and sequences will be compared in different groups of patients with low muscle strength (sarcopenia and myopathy in IKEM and elderly subjects in HFMRC). The outcome of the project will be the development of a comparable protocol at both sites, which will allow future multicentre studies including subjects with low physical strength, for whom dynamic testing cannot yet be performed with sufficient accuracy. The project aims are: 1. to compare the current examination protocols at both sites and to find the limiting parameters for the examination of persons with low exercise tolerance; 2. the development of comparable and appropriate protocols at both MR units, independent of instrumentation, for sufficient data quality to calculate muscle mitochondrial capacity, pH and other parameters characterizing muscle fitness even in persons with muscle weakness by appropriate examination protocol parameters; 3. comparison of data quality obtained from different magnetic fields (3T and 7T); 4. preparation of a joint project for the examination of selected patients at both sites.
