Caracterización de los mecanismos del daño vascular inducido por la apnea del sueño en la hipertensión nocturna y posibles dianas terapéuticas. Estudio HYPERSLEEP.
Description: OBJECTIVES: Nocturnal hypertension (NH) is strongly associated with cardiovascular risk. NH is difficult to identify because it frequently presents as masked hypertension, and a 24-hour ambulatory blood pressure monitoring (ABPM) is required to be diagnosed. Obstructive sleep apnea (OSA) is a frequent condition in NH. Nevertheless, the contribution of OSA to the increased cardiovascular risk in patients with NH has not been elucidated. We aim to evaluate, in patients with NH, the role of OSA and its treatment in the activation of pathophysiological drivers of cardiovascular disease. We will evaluate the potential deleterious effect of OSA in promoting accelerated endothelial dysfunction (ED), and the reversibility of this damage. METHODOLOGY: Observational, longitudinal, and multicentric study (7 hospitals) including patients from the Sleep Unit (bioSAOS cohort.n=296) and patients from the Hypertensive Unit (SARAH cohort.n=410). We will collect in patients with NH and with or without OSA, plasma samples and sociodemographic/anthropometric data. Baseline and post-follow up (12 months) data will be obtained. We will perform a sleep study and 24-h ABPM. Through machine learning processes, we will create a noninvasive model to define the profile of the patient with OSA and NH. From plasma samples, we will explore biomarker profiling and genomic pathways underlying ED. We will perform functional studies with in vitro models, to evaluate the potential of OSA for inducing ED mediated by the plasma exosomal cargo. We will characterize the plasma exosomal cargo containing molecules responsible for the induction of ED. We will also evaluate in vitro, the therapeutic potential mRNA therapeutics in the reversibility of ED. Finally, we will explore the effect of CPAP in blood pressure and ED biomarkers.
