Integrated Care for Older People with multimorbidity
Description: The constitution of the present European Network/Consortium is aimed to respond to an H2020 European call: “SC1-BHC-24-2020: Healthcare interventions for the management of the elderly multimorbid patient”. For this, we propose here to gather the most important European clinical research teams on geriatrics and the World Health Organization as the core elements of the Consortium. From this core Consortium elements, other partners (for example, a European, small-medium enterprise working on the remote monitoring of intrinsic capacities (eg, cognition, locomotion, hearing, vision, psychological, and vitality) and patients’ risk stratification) may potentially be added to in order to enrich and strengthen the Consortium. The research project we will propose is a pragmatic randomized controlled trial (RCT) to investigate the effectiveness and cost-effectiveness of a new, integrated healthcare pathway on intrinsic capacity (IC) and functional ability for older, multimorbid European citizens. The original aspects of this project are multiple, including (but not limited to): testing the efficacy of a healthcare pathway for multimorbid patients which is not disease-focused, but, instead, IC-centred; the project is positioned as a preventive strategy, leading to patients risk-stratification through the early detection of declines in ICs and timely interventions to avoid declines on functional ability and dependency; overtime, remote monitoring of IC using Information and Communication Technologies (ICT); self-management and monitoring of ICs. We believe our project has the potential to promote Healthy Aging by focusing on the management of intrinsic capacity levels and functional ability, without neglecting best practices related to the care of diseases. This Healthy Aging care pathway will strongly rely on self-management for the monitoring of intrinsic capacities, and will also involve formal and informal caregivers when appropriate. By delivering clinical guidelines for the care of this population, including with a well-identified and validated set of key quality performance indicators, our project will foster the use of best clinical practice for the multimorbid elderly around the world. Furthermore, this project will foster the use of social innovation in the monitoring of ICs, encouraging self-management and caregiver participation through ICT. The participation of the WHO as a partner of this network will strengthen the capacity of the Consortium to disseminate the project’s findings; this will probably facilitate the real life implementation of recommendations issued from the project. Our project will therefore provide scientific and economic evidence about the usefulness of the new health aging pathway. By providing a thorough health economic analysis, this project will be a cornerstone not only for clinical researchers and healthcare providers dealing with the multimorbid elderly, but also for policy makers and health authorities. In sum, our project has a large potential for changing healthcare practices, including in terms of public health policies.
