La classe II division 2 est-elle un facteur de risque dans la dysfonction craniomandibulaire ?

Abstract: Class II, division 2 is a clinical entity which is the subject of plenty of theories and debates about the impact of the temporomandibular joint (TMJ) in temporomandibular disorders (TMD). It appears that the « locked occlusion » provoques a mandibular retrusion that leads to articular compression. A literature review was conducted from the electronic databases of PubMed, Scopus, Google Schoolar and ScienceDirect from 1989 to 2015. This research helped identify 8 articles which specifically concerned the Class II division 2. Due to the lack of articles published on this topic, a second piece of research was carried out which examined the individual traits of Class II division 2 when considering 20–45 year old individuals. From the conclusions of these studies, it’s possible that anatomical characteristics play a role in the temporomandibular disorders, specialy when people are stressed, psychologicaly weak and geneticaly predisposed (hypermobility of the ligaments for example). The severity of the clinical form is dependent on the predominant musculature which plays a role in the therapeutic standpoints, and it is crucial to consider in order to avoid recurrences. For example, the strong activity of the posterior temporal muscle influences the effects of this malocclusion by maintaining the retrusive position of the mandibule. It’s necessary to coordinate all the information of the diagnosis to manage the TMD.
