Abstract: Compensation for subject and time variation in blood flow measurement requires standardisation, this has yet to be adequately performed using raw PD-US image data. We propose to extend a validated 2D blood flow estimation method, FMBV, into 3D and determine the maternal blood flow around the utero-placental interface (UPI). 143 women were scanned with 3D PD-US at 11–13 weeks (GE Voluson E8) using fixed machine settings and individualised sub-noise gain (as previously published). Placental volume was estimated offline by a validated semi-automated method and the UPI was identified. A Euclidean distance transform was used to automatically determine volumes of interest (VOIs) relative to the UPI thereby avoiding manual sampling error. The algorithm then determined the FMBV std. reference value from the maternal vasculature 10 mm below the UPI (shown to contain std. reference values equivalent to the uterine artery at the same depth). FMBV was calculated for the specified VOIs according to the previously published algorithm which corrects for the rouleaux formation in maternal blood. 3D FMBV values were obtained for all subjects (Table 1). The values were significantly greater for the VOI 5 mm into the uterus (containing the spiral arteries) compared to 5 mm into the placenta (the intervillous space) (P < 0.01; Mann Whitney U test). OC18.03: Table We have demonstrated that FMBV can be successfully extended into 3D using offline semi-automated image processing. By measurement relative to the UPI, we sampled only the maternal circulation since fetal blood differs in rheology. This produced biologically concordant results concordant with histological studies. 3D-FMBV has many advantages over current methods; standardisation allows intra/inter patient comparison; automation reduces sampling and operator error; and the ability to process the raw image data from any type of 3D-US machine makes it widely applicable.
