Abstract: Aim: To evaluate whether PDA-phone telemonitoring improves the outcome of Functional Insulin Therapy (FIT) training courses, compared to conventional care, for type 1 diabetics. Method: The study comprised a prospective randomized trial on 59 adults split in two parallel similar groups: The 'PDA Group' received a PDA-phone and a full month of phone consultations with a physician, whereas the Control Group only received standard paper support. The judgment criteria was evolution of HbA1c between start and 3 months in the two groups. Findings: The two groups were clinically and metabolically similar at inclusion (HbA1c 7.9 % [7.4; 8.7]). The FIT training course significantly decreased HbA1c levels at 3 months in the total population (8.13 ± 1.25 vs 7.80 ± 0.98, p=0.0252 ANOVA). However, the decrease between the PDA Group (8.10 ± 1.19 vs 7.73 ± 0.93 %) and Control Group (8.0 ± 1.14 vs 7.88 ± 1.04) is not statistically significant (p=0.2535). The HbA1c levels tend to decrease more in the PDA Group (-0.30% [-0.50; 0.30], p=0.0999) than in the Control Group (-0.05% [-0.55; 0.35], p=0.3681). The improvement of HbA1c at 3 months tends to be higher for the patients who are not well balanced (HbA1c > 7.5%) for the PDA group (-0.4% [-0.9; 0.2]) than for the control group (0% [-0.9; 0.2], p= 0.2561). Conclusion: the reinforcement of FIT training course with PDA-phone telemonitoring tends to improve the metabolic balance of patients, but the lack of statistical significance due to the small sample size. This study contributes to illustrate the place of the tele-medicine in the therapeutic treatment of type 1 diabetes.
