OP29.04: Placental vascularization by three-dimensional power Doppler in the twin-to-twin transfusion syndrome

Year: 2009
Type: conference-abstract
Abstract: Twin-to-twin transfusion syndrome (TTTS) affects 10–15% of monochorionic diamniotic pregnancies due to unbalanced intertwin blood flow through placental vascular anastomoses. Fetoscopic laser photocoagulation of vascular anastomoses on the placental surface has become a standard treatment. The three dimensional ultrasound has been in use to better understand anatomy and pathophysiology of different organs. Power Doppler is used to evaluate the vascular tree and the quantification of total blood flow in the placenta. The quantifying blood flow method using 3D ultrasound is used currently in the diagnosis of ovary pathology. The use of vascular indices such as vascularization index (VI), flow index (FI) and vascularization-flow index (VFI) in placenta as a quantitative method of evaluation of vascular anatomy and pathophysicology has been introduced. The aim of this study is to compare the changes in vascular indices before and after the laser photocoagulation. We performed a prospective observational study of all consecutive cases that underwent laser surgery for TTTS at Texas Children's Fetal Center between November 2008 to march 2009. We report the data in 23 consecutives cases of TTTS in which we performed the evaluation of the vascularization of the placenta pre and post laser ablation. The analysis of the placental vascularization and the vascularization Indices by three-dimensional power Doppler is useful to make an evaluation of the placental vascularization before and after the Laser procedure in the twin-to-twin transfusion syndrome, however, further studies are required to confirm the results and the potential uses of this instrument in the assessment of this pathology.
