Bridging to cardiac transplantation: a clinical laboratory for the development of mechanical alternatives to transplantation

Year: 1999
Type: article
Abstract: THE FIRST attempts at mechanical cardiac assistance were made almost 70 years ago, and subsequent work culminated in the development of the heart-lung machine in 1953. The next successful circulatory assist device, the intraaortic balloon pump, was developed in 1961 and first applied clinically in 1967. The first patient was bridged to cardiac transplantation by a ventricular assist device in 1978 at the Texas Heart Institute, surviving for 5 days on a pneumatically powered LVAD until a donor organ became available. Recently, new prosthetic materials, engineering advances, and increasing clinical experience has resulted in a new generation of ventricular assist systems. These devices have been used successfully for both brief and longterm support. Although currently used widely as bridges to transplantation, such devices will almost certainly become available in the near future as a long-term form of cardiac replacement.
