Abstract: A 58-year-old African-American male was found to have proteinuria on routine health screening. Evaluation revealed a monoclonal “M” spike, 2% plasma cell population on bone marrow biopsy and a negative skeletal survey. He was given a diagnosis of monoclonal gammopathy of undetermined significance (MGUS). A kidney biopsy was performed and revealed focal segmental glomerulosclerosis (FSGS) with no evidence of light chain disease. He subsequently underwent a one haplotype–matched living related kidney transplant from his son.
