P03.10: Hand positions in fetuses with trisomy 18

Year: 2005
Type: conference-abstract
Abstract: To examine the incidence of abnormal hand positioning in trisomy 18 fetuses in antenatal ultrasound study. Retrospective review of ultrasound images of hand position of fetuses with trisomy 18 scanned in our hospital from 1998 through 2004. Hand position was classified as either (A) clenched, (B) more closed than open, (C) more open than closed, (D) open. 37 cases of trisomy 18 were confirmed by cytogenetic studies. 16 were specimens from first trimester. 21 cases had scanned performed after 15 weeks. Hand images were available from 15 cases. 6/15 (40%) had clenched hands (A). 3/15 (20%) hands were partially clenched (B). 4/15 (27%) fetal hands were more open than closed (C). 2/15 (13%) fetal hands were open (D). One fetus in group B and one from group C had no anatomical nor biometric anomaly on the ultrasound examination. If we assume that hand images were not available in 6 cases because normal views could not be obtained at the time of study, 6/21 (28.6%) fetuses with trisomy 18 scanned in this series had either more open than closed hands or open hands. Clenched fetal hands are not universally present in Trisomy 18 fetuses. This has important implication when studying fetuses at risk for Trisomy 18.
