Abstract: To the Editor: We thank Dr. Miller for his interest in our study of long-term seizure and quality-of-life outcome after epilepsy surgery for pathologically proven hippocampal sclerosis (1). Dr. Miller draws attention to two previous publications that show similar favorable outcomes with respect to seizures in this patient population. Our study extends the findings of the previous studies by documenting the improved quality of life in patients who are rendered seizure free after the surgery. We agree that the key area for future research in this area is to elucidate why some patients with hippocampal sclerosis have a poor outcome after surgery. Our group has examined this issue in two previous studies and has shown that clinical factors do not predict seizure recurrence, and if magnetic resonance imaging shows hippocampal sclerosis or a foreign-tissue lesion, and ictal EEG is concordant or nonlateralizing, then results of other presurgical evaluations do not predict seizure outcome (2,3). The questions raised by Dr Miller remain unanswered.
