Abstract: In the surgical treatment of patellar instability, an elevated tibial tubercle trochlear groove (TTTG) distance is considered a contributor to malalignment. Some surgeons use a TTTG distance greater than 15mm as a threshold for diagnosing excessive lateralization and therefore considering a medializing tibial tubercle osteotomy. Recent studies suggest that TTTG distance increases at terminal extension, although the clinical significance of this has not been shown. We utilized a dynamic kinematic computed tomography scanning method to measure TTTG distances at various flexion angles in patients with recurrent patellar instability, and correlated this to measurements of bisect offset (a measurement of patellar lateralization) and tilt.
