Abstract: In this report the results of a background study of the care for people with a chronic condition are given. The study is performed by the NIVEL and RIVM at the request of the Health Care Inspectorate. It serves as a basis for the 'State of the Health Care 2003: Coordination of care for the chronically ill'. The study has a special focus on the coordination of care between health care providers.
