Abstract: A 61-year-old right-handed musician presented with a generalized tonic-clonic seizure. Two years previously, he had a similar episode. His medical history was significant for mild asthma and 45 pack-years of smoking.Magnetic resonance imaging (MRI) of the brain showed a contrast-enhancing mass in the right parietal lobe (Fig 1A) with surrounding edema on fluid attenuation inversion recovery (FLAIR) imaging (Fig 1B).Computed tomography of the thorax revealed a 2.1-cm right-upper lobe suprahilar mass, which biopsy demonstrated was poorly differentiated squamous cell carcinoma.Resection of the right parietal mass proved to be metastasis of the non-small-cell lung cancer. He then underwent 18-Gy stereotactic radiosurgery in one fraction to the right parietal operative bed.Systemically, he also received four cycles of platinum-based chemotherapy followed by consolidative thoracic radiotherapy. One year after Fig 2.
