Abstract: Rehabilitacji Kardiologicznej Instytutu Kardiologii w Warszawie Value of exercise seismocardiography in the diagnosis of ischemic heart disease Introduction: Seismocardiography (SCG) is a non invasive method of detecting coronary artery disease. It combines technology from fields of seismology and signal analysis to accurately evaluate the biomechanical function of the heart. An exercise SCG test is performed in conjunction with standard exercise ECG test (EXT).The aim of the study: The aim of the study was to evaluate a potential use of SCG in diagnosis of ischemia in patients with ischemic heart disease pertaining to an electrocardiographic and scintigraphic exercise test.Material and methods: 40 male patients with CHD, average age of 49.2 years, were subjected to full-scaled analysis.All patients were scheduled for following exercise tests: EXT, SCG and SPECT.The following parameters were correlated: sensitivity, specificity, positive diagnostic value (PDV), negative diagnostic value (NDV) as well as the accuracy of these three methods.The main reference point was coronarography.Results: The sensitivity of SCG prevailed over the sensitivity of EXT (p < 0.01).Diagnostic effectiveness indexes, that is sensitivity, specificity, PDV, NDV and accuracy of SPECT did not differ considerably compared to SCG.There were no substantial differences either in the whole group or in subgroups with or without MI, with stenosis in 1, 2 or 3 arteries and in case of irreversible and reversible losses perfusion in scintigram.In comparison with EXT, SPECT turned out to be a far more sensitive method in the whole group of 40 persons (p < 0.001) as well as in the subgroups with MI (p < 0.004) and without MI (p < 0.04) and in case of a single vessel stenosis (p < 0.02).Conclusions: Seismocardiography is more sensitive method of diagnosing ischemia in IHD compared to EXT.The sensitivity of SCG can be compared to the sensitivity of the scintigraphic exercise test.(Folia Cardiol.2001; 8: 381-388) ischemic heart disease, seismocardiography, exercise test, SPECT
