Abstract: Childhood obesity is a risk factor for asthma, but the influence of obesity on asthma severity is less clear. This study characterized body composition, obesity related comorbidities, and traditional risk factors for asthma in a cohort of children. We hypothesized that children with asthma and obesity would have distinct risk factors and co-morbidities, and that obesity and related co-morbidities, including sleep disordered breathing (SDB) and metabolic syndrome, negatively influence asthma severity.Asthma severity was determined after one year of prospective follow up.Obesity was common in study participants. While obesity was not associated with asthma severity, SDB was associated with a 4.85 increased odds of severe asthma (95% CI 1.94-12.10,p=<0.001).After adjusting for BMI z-score, race, and sex, the association remained significant (OR 3.62, 95% 1.26-10.4,p=0.02).Further studies will be needed to determine the mechanisms underlying this association, and whether treatment of SDB improves asthma outcomes.
