Abstract: Cerebrospinal fluid (CSF) tau, phosphorylated tau (p-tau), and amyloid beta (Aβ42) protein levels are among the most well validated Alzheimer's Disease (AD) biomarkers. F18-T807 is a newly developed radioligand that allows for in vivo imaging of tau pathology. Comparison of T807 binding to well established CSF AD biomarkers is a key step in the characterization of this new radioligand. F18-T807 PET and CSF assays of total tau, p-tau, and Aβ42 were obtained in 31 cognitively normal participants in the Harvard Aging Brain Study (mean age: 76.2y±6.4y; PiB FLR: 1.19±0.23). Linear regression models were used to examine relationships between CSF measures and regional T807 SUVR. After controlling for sex and structural measurements, CSF tau and p-tau were significantly correlated with T807 SUVR in entorhinal (tau: r=0.54, p=0.002; p-tau r=0.43, p=0.015), parahippocampal (tau: r=0.61, p<0.001; p-tau: 0.50, p=0.004) and inferior temporal (tau: r=0.52, p=0.003; p-tau: r=0.56, p=0.001) regions. These correlations persisted after controlling for age and CSF Aβ42. The relationship of CSF tau to T807 SUVR became progressively weaker when moving from inferior temporal to middle temporal (r=0.36, p=0.045) to superior temporal (r = 0.25, p = 0.18) regions, areas with progressively less tau pathology in the cognitively normal elderly. CSF p-tau remained significantly correlated with T807 SUVR in inferior, middle (r=0.45, p = 0.012), and superior temporal (r=0.43, p=0.017) regions. CSF Aβ42 correlated with T807 SUVR in temporal neocortical regions, with the highest correlation seen in the inferior temporal cortex (r=-0.46, p=0.010). CSF Aβ42 was not significantly correlated with entorhinal (r=-0.17, p=0.37) or parahippocampal (r=-0.15, p=0.42) T807 SUVR. T807 binding in entorhinal, parahippocampal, and inferior temporal regions was correlated with CSF tau and p-tau levels in cognitively normal elderly with tau pathology largely restricted to temporal lobe regions. These data support the link between T807-PET and CSF-based measures of tau pathology. Correlation between CSF measures and F18-T807 SUVR. T807 binding from the parahippocampus region-of-interest (ROI; left) significantly correlated with tau (p<0.001) and p-tau (p=0.004) CSF protein levels in 31 cognitively-normal elderly individuals. T807 SUVR was not significantly correlated with CSF Ap42 in this parahippocampal ROI.
