Abstract: Fifteen patients undergoing resection of carcinoma of the papilla of Vater who did not present with jaundice were compared with 40 same patients presented with jaundice for clinicopathological findings. Of the 15 patients without jaundice, 6 patients had no symptoms. As much as 10 patients without jaundice were first detected having abnormal findings by ultrasound. There was no relation between the presence of jaundice and macroscopic morphorogy of the primary tumor, or tumor size. Patients without jaundice had tumors invading beyond the muscle of Oddi, duodenum invasion, pancreas invasion, or lymph node involvement in less frequency than that in patients with jaundice. The prognosis of patients without jaundice was significantly better than that of others between one and 9 years after the operation. We think that detecting the carcinoma of the papilla of Vater before occurrence of jaundice contributes to improve the prognosis of resected cases.
