Abstract: activity of matrix metalloproteinases (MMP's), specifically MMP-9 (gelatinase B) in the urine of controls and IC/PBS patients.METHODS: In a cross-sectional study, spot urine specimens were collected from 7 IC/PBS patients diagnosed according to NIDDK criteria and 5 healthy asymptomatic controls.MMP-9 levels in un-concentrated urine specimens was measured using the Abcam ab100610-MMP9 Human ELISA Kit.The MMP-9 protease activity was analyzed via gel zymography after 80 fold concentration of urine specimens using Amicon Ultra-4 10 KDa centrifugal filters.RESULTS: MMP-9 levels were significantly higher in the urine of IC/PBS group compared to the control group (6.02 Æ 4.13 ng/mL vs 0.82 Æ 0.26 ng/mL; p¼0.02).Zymogram analysis using the AlphaEaseFC software showed that the gelatinase activity corresponding to the high molecular weight MMP-9 (HMW-MMP9; ~125 kDa) was 3.87 Æ 0.31-fold higher in urine of IC/PBS relative to controls (p ¼ 0.002).The representative gel picture shows the band for HMW-MMP9 in controls (Lane 1&2) and for IC/PBS patients (Lane 3&4).The HMW-MMP9 represents the complex of MMP-9 with neutrophil gelatinase-associated lipocalin, which protects MMP-9 from degradation.CONCLUSIONS: Our findings demonstrate the increased urine levels of total MMP-9 and increased enzymatic activity of HMW-MMP9 in IC/PBS patients.Confirmation of increased MMP-9 activity by gelatin zymography was critical as MMP-9 is expressed in the latent proenzyme form and requires enzymatic cleavage for activation. One of the substrates of MMP-9 is zona occludens-1 (ZO-1), which plays an important role in bladder permeability. Therefore, increased MMP-9 activity in IC/PBS supports the reports of decreased ZO-1 in the IC/PBS bladder as well as the hypothesis of a compromised bladder permeability barrier in IC/PBS.
