Abstract: 下咽頭食道全摘出後の再建は頭頸部外科医にとって難手術の一つである。その異なる3再建術式の利点・欠点を明らかにするため，出血量・手術時間，合併症，術後機能を比較した。対象は54～72歳男性，下咽頭癌・頸胸境界部食道癌17例で，術式は胃管（n＝5），延長胃管（n＝7），遊離空腸＋胃管（n＝5）であった。各術式で出血量・手術時間に差はなく，胃管で部分壊死，空腸＋胃管で小瘻孔が各1例に生じたが，延長胃管では切除関連合併症が多かったものの，縫合不全はなかった。摂食内容に差はなく，食道発声は延長胃管と空腸＋胃管の各1例で可能になった。この結果から3術式に顕著な差はないが，全身・社会的要因に基づき術式を選択すると良いと考えた。
