We sought to evaluate the predictive value of the mutational status of these genes on survival in patients with Stage IV ccRCC (American Joint Committee on Cancer) at presentation who underwent cytoreductive nephrectomy. METHODS After obtaining institutional review board approval, we evaluated patients with ccRCC and available genomic and clinical data from our prospectively collected institutional database (n=85 patients) and several publicly available cohorts (n = 109), including The Cancer Genome Atlas, International Cancer Genome Consortium and University of Tokyo. We identified 194 patients with Stage IV ccRCC on presentation, and we stratified patients into three groups based on the mutational status of BAP1 and PBRM1. A fourth group, BAP1 mutant (BAP1mt) and PBRM1 mutant (PBRM1mt) is extremely rare and these patients were grouped with the BAP1mt/PBRM1mt-wt patients. The cancer-specific survival (CSS) of each group was compared using the log-rank test. RESULTS Of the 194 patients, we found 27 patients (13.9%) who had mutant BAP1mt and PBRM1mt-wt, 73 patients (37.6%) with PBRM1mt/BAP1wt, and 94 patients (48.4%) with BAP1wt/PBRM1wt. Median CSS in months was 18.8 for BAP1mt/PBRM1mt-wt, 40.3 for PBRM1mt/BAP1wt and 33.6 for BAP1wt/PBRM1wt. The CSS for these three groups can be seen in Figure 1. Log-rank testing shows the curves to be significantly different with a p-value of 0.004. CONCLUSIONS BAP1 and PBRM1 mutational status gives us three groups that demonstrate prognostic significance in patients with stage IV ccRCC undergoing cytoreductive nephrectomy. This stratification may aid in clinical decisions to initiate treatment after surgery, even in those with low volume or stable metastatic disease burden. © 2016FiguresReferencesRelatedDetails Volume 195Issue 4SApril 2016Page: e1032-e1033 Advertisement Copyright & Permissions© 2016MetricsAuthor Information Brandon J Manley More articles by this author Jozefina Casuscelli More articles by this author Daniel M Tennenbaum More articles by this author Maria F Becerra More articles by this author Almedina Redzematovic More articles by this author Maria E Arcila More articles by this author Paul Russo More articles by this author Jonathan A.
