Determining the optimal treatment for advanced bladder cancer.

Abstract: Muscle-invasive transitional cell carcinoma (TCC) portends a poor prognosis. In Europe, physicians have favored radiation and chemotherapy, whereas treatment in North America has been largely directed toward surgical extirpation. Although radical cystectomy remains the gold standard for the surgical treatment of muscle-invasive TCC, recent reports have suggested that aggressive transurethral resection alone may be sufficient in some patients. Studies that examine the effectiveness of these two different surgical strategies for muscle-invasive TCC are reviewed. Although chemotherapy is the therapy of choice for patients with advanced TCC, the most effective combination of cytotoxic agents is unclear. Small phase II trials have suggested that the combination of gemcitabine and cisplatin may have efficacy similar to the standard methotrexate, vinblastine, doxorubicin, and cisplatin, while reducing toxicity. The results of a large phase III multicenter trail comparing these two treatments are also reviewed.
