Chemical induction of ovulation with clomiphene and its mechanism.

Year: 1966
Type: article
Abstract: Much has been reported on the ovulatory action of clomiphene citrate (MRL-41), a non-steroidal synthetic compound, in human female. The present study was undertaken to pursue observation of 161 cycles of 104 women treated by clomiphene citrate. Main purpose of this study was to clarify the indication for clomiphene treatment, influence on the sexual function during administration as well as the mechanism of action of clomiphene in induction of ovulation. It is concluded that the cases to be indicated for this treatment are those with positive progesterone test, i.e. with sufficient endogeouns estrogen, and with no organic disorders in the other endocrine glands. In most of these women under clomiphene treatment, antiestrogenic action was apparent, and the common pattern of the hormonal change outlined by estimation of daily urinary excretion was that FSH is released first and is followed by increase of estrogen and ovulatory LH. A few cases with low estrogen, however, ovulated. This fact seemed to indicate some unknown mechanism of action of clomiphene in induction of ovulation. But, in general, the data obtained strongly suggest that the site of action of clomiphene is the hypothalamico-pituitary ovarian axis. Various BBT patterns were also discussed as one of the manifestations which shows the complex action of clomiphene on sexual function.
