Ensuring Successful Transitions and Appropriate Survivorship Care for Pediatric Stem Cell Transplant Recipients By Leveraging the Electronic Medical Record (EMR) in a Community Based, Hybrid Type Survivorship Model

Type: article
Abstract: Childhood survivors of stem cell transplant have a significant risk of developing chronic medical conditions due to exposure to high dose chemotherapy and total body irradiation. Providing timely and appropriate survivorship care is vital to early detection and ensuring good outcomes. These is even more challenging as children transition into adulthood and are vulnerable to inadequate survivorship care. We report a community-based, hybrid type model of survivorship care leveraging the EPICTM EMR to work in parallel with adult primary care providers to deliver convenient and effective population care management.
