Abstract: For patients with bone marrow failure (BMF) lacking an HLA matched related donor (MRD), traditional approaches for unrelated donor stem cell transplantation have utilized bone marrow as a stem source due to high rates of GvHD and poorer survival with T cell replete peripheral blood stem cell transplantation (PSCT). However, PSCT has several potential advantages in patients with BMF, including higher stem cell doses to prevent graft failure and faster rates of engraftment. We present a retrospective analysis of patients (pts) with BMF who received PSCT with partial T cell depletion (pTCD) using a strategy of CD3/CD19 depletion with targeted T cell add-back, chosen to balance the risk of GvHD seen with T cell replete PSCT with the risks of graft failure and viral infections seen with full T cell depletion.
