A patient with Laron syndrome and diabetes: case study

Year: 2012
Type: article
Study design: Case Report
Abstract: Laron syndrome is a rare genetic disease, characterised by dwarfi sm, typical facial features and central obesity. In the basis of pathomechanism lies resistance of growth hormone receptor which leads to insuffi ciency of IGF 1 (IGF 1, insulin like growth factor 1). Within the population of patients with Laron syndrome, it has been observed that they are less likely to develop malignant tumour and diabetes mellitus. It is probably caused by IGF1 insuffi ciency. If some patients develop diabetes mellitus, it is caused by insulin resistance growing with age. In this report a case of 20 year old female with Larone syndrome and diabetes is presented. The patient developed absolute insulin insuffi ciency. Patient was admitted because of uncontrolled diabetes with glucose levels ranging from 30 mg/dl (1.67 mmol/l) to about 1000 mg/dl (55.5 mmol/l). During the hospitalisation the patient’s insulin therapy has been modifi ed. Due to highly probable insulin resistance compound, it has been decided to start the treatment with metformin, aiming to gain better glucose control.
