Test Dose Pharmacokinetics to Predict Melphalan Dosing in Children Undergoing Hematopoietic Stem Cell Transplant (HSCT) with Organ Impairment☆

Type: article
Abstract: Background: Melphalan is commonly used in reduced intensity conditioning (RIC) regimens in children undergoing allogeneic HSCT for non-malignant disorders and causes toxicity. Pharmacokinetic (PK) data for melphalan in children are limited, especially in RIC HSCT for non-malignant disorders. A novel paper spray tandem mass spectrometry (PS-MS/MS) assay allows quantitative analysis of melphalan from a small amount of blood within minutes without requiring special sample processing. We hypothesized that use of a test dose would allow dose adjustment of melphalan in real-time in children with organ impairment.
