Impact of CYP3A5/CYP2C19 Pharmacogenetics on Clinical Outcomes after Allogeneic Hematopoietic Stem Cell Transplantation (HCT) Using Tacrolimus/Sirolimus (Tac/Sir) as Graft Versus Host Disease (GVHD) Prophylaxis

Abstract: Genetic polymorphisms implicated in drug absorption, distribution, metabolism, and excretion (ADME) determine the pharmacokinetic variability of many drugs and are recognized as important factors determining the success or failure of treatments. Using the iPLEX® ADME PGx Panel (188 single nucleotide polymorphism [SNP] and 12 copy number variant [CNV] assays in 36 genes), we previously studied 177 patients who underwent HCT using tac/sir-based GVHD prophylaxis. In this earlier pilot study, we demonstrated the effect of certain SNPs (CYP3A5, ABCB1) on tac/sir blood level as well as acute GVHD (Khaled et al.
