Abstract: Role of adrenal fetal zone and it's vascularity to predict successful induction of labour. This is a retrospective cohort study of over 250 singleton pregnancies where all the patients referred to our centre for ultrasound scan from multiple referring centres were screened for fetal adrenal gland. The ratio of total gland volume and its inner zone were derived. The vascularity of the middle adrenal artery and perfusion of the adrenal gland were determined and compared against the labour outcomes of all these patients, while ROC curve analysis was used to assess the predictive value. Adjusting for lie and liquor at delivery, the overall size of adrenal gland and its inner zone was one of the single most important criteria in determining the outcome of induction of labour. The peak systolic velocity of middle adrenal artery was the second important determinant in the outcome. Those fetus with larger inner zone of the adrenal and higher PSV in middle adrenal artery were commonly associated with successful vaginal delivery after induction of labor. The vascularisation indices of adrenal gland like FI, VFI, VI did not contribute significantly to the outcomes. The fetal adrenal zone and its blood flow indicate activity and contribute significantly to the successful outcome of induction of labour.
