Abstract: UROLOGy® recurrences, clinical and pathologic stage, and current disease status.Patients were divided into two groups -those having cystectomy before 1998 (median year surgery), and those after 1998.Trends regarding time from diagnosis to surgery, number of intravesical therapies, recurrences, and upstaging of disease from clinical to pathologic stage were analyzed using two-sample !-tests with 95% confidence intervals. In addition, Kaplan-Meier analysis with log-rank test was performed to determine disease-free survival rates.RESULTS: 94 patients were included in the final analysis with a median age of 69.1 years at time of cystectomy.Prior to 1998, 27 of 38 patients (71.1 %) proceeded directly to cystectomy without intravesical therapy versus 19 of 41 (46.3%) after 1998 (p=0.046).The mean number of intravesical treatments per patient was 0.68 prior to 1998 and 1.17 after 1998 (p=0.057).There was no statistical difference between the number of recurrences or time to cystectomy from original diagnosis between groups.Patients having cystectomy before 1998 had a 74% disease-free survival after cystectomy at 5 yrs versus 54% for those after 1998 (p=0.038).CONCLUSIONS: In the past 15 years, our experience indicates that patients with T1 superficial bladder cancer are now more likely to receive intravesical therapy. In addition, we noted disease-free survival to be better prior to 1998 in this group. In very few other cancers has disease-free survival decreased over time.We postulate the decrease in survival may be related to the increase in intravesical therapy.
