Predictors of Gleason upgrading at the time of radical prostatectomy (RP) after fusion biopsy have not been well defined. We sought to determine factors associated with concordance of FBx and RP. METHODS We conducted a retrospective analysis of men who underwent FBx and RP at our institution between 2012 to 2017. Clinical, multi-parametric magnetic resonance imaging (mp-MRI) and histopathology features were reviewed for each subject. Univariable and step-wise multivariable regression analysis was performed to identify factors associated with Gleason upgrading in RP specimens. RESULTS 100 men were identified. Median age was 65.7 (interquartile range [IQR] 60.5-70.1) years. Median PSA was 6.4 (IQR 4.8-10.2) ng/mL. Median PSA density was 0.17 (IQR 0.12-0.27) ng/mL/cc. Gleason upgrading and downgrading rates were 28% and 22%, respectively. 35.7% of upgraded men had upgrading of the primary Gleason pattern. Age, BMI, PSA density, PI-RADS and number of lesions on mp-MRI were not associated with Gleason upgrading in univariable analysis and step-wise multivariable regression. CONCLUSIONS In our single center study of patients undergoing fusion biopsy and radical prostatectomy, Gleason upgrading rates were found to be similar to standard ultrasound guided prostate biopsies. We did not identify any clinical features predictive of upgrading on final radical prostatectomy specimen. © 2018FiguresReferencesRelatedDetails Volume 199Issue 4SApril 2018Page: e814-e815 Advertisement Copyright & Permissions© 2018MetricsAuthor Information Neal A.
