Abstract: In Brief Liver intraportal transplantation of pancreatic islets has limitations. Other alternative anatomical implantations are disappointing due to low vascularization. Islet implantation in subcutaneous, and as described in this issue of Transplantation, hepatic pre-existing vascular bed supply show satisfactory results. The liver is preferable since has better insulin exposure through portal circulation. Future studies using this new implantation site should include the use of allogeneic islets and immunosuppression.
