Abstract: Failure mode and effects analysis (FMEA) is a powerful tool for identifying and assessing potential failures. The tool has become increasingly important in systems where there are many risks. The common FMEA uses Risk Priority Number (RPN) to evaluate the risk level of a component or process. The RPN is a mathematical product of the severity (S), the occurrence (O) and the detection (D). The traditional FMEA has been a well-accepted safety analysis method; however, it suffers from several drawbacks. The most critical disadvantage of the traditional FMEA is that various sets of (S), (O) and (D) may produce an identical value of RPN; however, the risk implication may be totally different. In this paper is the proposal of a new methodological approach called „RRI - Risk Ranking Index“ to overcome these shortcomings. The index proposed is based on the MUT - Multiattribute Utility Theory.
