Abstract: hypertrophy and attenuates muscle weakness. It may increase urethral pressure by facilitating recovery of injured muscles in a dual injury (DI) SUI rat model.METHODS: Rats were anesthetized and underwent pudendal nerve crush (PNC) followed by vaginal distension (VD).One week after surgery, each rat began treatment via subcutaneous injection (0.3ml/ rat) once daily in a blinded fashion with either bimagrumab (DIþBim), clenbuterol (DIþCle), 5-hydroxybenzothiazolone derivative, a novel b2 adrenoceptor agonist with superior skeletal muscle selectivity in rodents compared to clenbuterol (DIþ5-HOB), or PBS (DIþSham).Sham rats underwent sham PNCþVD and received PBS (SI).Body weight was monitored weekly after injury. After two weeks of treatment, the rats were anesthetized for leak point pressure (LPP) and external urethral sphincter (EUS) electromyography (EMG) recordings.Hind limb skeletal muscles (quadriceps, gastrocnemius complex, tibialis anterior) were dissected and weighed.RESULTS: At the end of 2 weeks of treatment, all 3 treatment groups (DIþBim, DIþCle and DIþ5-HOB) had gained significantly more weight (23.49%Æ1.93%,14.20%Æ1.37%and 15.87%Æ1.54%,respectively) than the DIþsham group (2.73%Æ0.64%).All three treatments also increased the weight of all three hind limb muscles compared to both sham treated (DIþsham) and sham-injured (SI) rats.Rats treated with Bimagrumab had the greatest weight increase in all 3 muscles.LPP in the DIþsham (33.5Æ4.0 cmH 2 O), DIþ5-HOB (41.3Æ3.7 cmH 2 O), and DIþClen (29.9Æ2.9 cm H 2 O) groups were significantly decreased compared to SI rats (57.8Æ2.6 cm H 2 O).In contrast, LPP in the DIþBim group (46.8Æ3.3cmH 2 O) was not significantly different from LPP of SI rats but was significantly higher than LPP of DIþsham and DIþClen rats.CONCLUSIONS: Blockade of activin type II receptors with Bimagrumab has the strongest effect on muscle growth and could be a promising treatment to restore continence by increasing LPP.Given that the drugs were administrated systemically, pelvic floor muscles should also be affected by the muscle growth in addition to the EUS.The morphological changes of continence-related muscles need to be further investigated.
