Abstract: Rapid eye movement (REM) sleep behavior disorder (RBD) is a parasomnia associated with increased muscle tone during REM sleep. 1 It strongly marks neurodegenerative synucleinopathies, including Parkinson's disease and dementia with Lewy bodies. 2 Many narcolepsy patients have RBD symptoms.3 It has been suggested that the pathophysiology of RBD in narcolepsy differs from idiopathic RBD, perhaps being related to the instability of sleep stages. Moreover, patients with narcolepsy show fewer degenerative disease signs.4 This would imply that narcolepsy-RBD should not be associated with neurodegenerative disease. Here we report a patient with narcolepsy who developed neurodegenerative synucleinopathy, albeit with characteristics that seemed to suggest 2 independent pathophysiologies.
