Abstract: Patients on chronic haemodialysis usually have several types of complications in their access. A rare one is related to arteriopathy in the upper limbs. We present this case with brachial artery sub occlusion fistula from radiocephalic vein to radial artery; The patient had low blood flow in the fistula and hand ischemia with tissue lesions. The challenge was to solve the arterial flow of the fistula and the ischemia of the hand. Patient 68 years, associated with smoking, high blood pressure, non-diabetes, 5 years in haemodialysis; cannot dialyze due to low flow in the arteriovenous fistula, and with ischemia finger injuries. Fistulography was performed with arteriography, which identifies a severe lesion in the distal third of the brachial artery, near the radial and ulnar bifurcation. Side to side anastomosis of brachial artery is decided before stenosis with basilic vein, connected with radial and cephalic vein through the venous M of the elbow. The radial vein was distally served as a bypass for the treatment of hand ischemia and the cephalic vein increased the flow to be able to dialyze The resolution of hemodialysis accesses is not always as regulated as in most surgeries, it often requires sharpening the artisanal resources of the surgical team to the maximum, adapting to each particular patient
