However, increased appreciation of the long-term morbidity profile associated with these treatments has raised concerns. Primary RPLND in these patients may allow for more precise staging and can potentially avoid or reduce the exposure to chemotherapy or radiation. METHODS: Following IRB approval, all patients with testicular seminoma undergoing primary RPLND for the treatment of suspected retroperitoneal node(s) were retrospectively identified from a single institution’s prospectively collected database. Patient demographics, oncologic staging information, peri-operative details, pathologic diagnoses, and adjuvant treatment and recurrence outcomes were collected and described. RESULTS: In total, 12 patients were identified (Table 1). Of these, 9 were CSI seminoma that recurred in the RP during surveillance and 3 were CSII seminoma at presentation. For relapsing CSI patients, all were treated with RPLND for “early recurrences” (7 mo median time to recurrence) with median pre-operative RP node size 1.9cm (cN1=5, cN2=4). For CSII patients, 2 patients were cN1 and 1 was cN2. All RPLNDs were bilateral template. Median number of LN removed was 61 (range 27-159) and median number of positive nodes was 2 (range 1-6). Two patients were pN0, one patient had teratoma (pN1) and all others were seminoma (1 pN1, 8 pN2). In total, 7 patients were managed with post-RPLND observation and 5 received chemotherapy (all EPx2). To date, none have recurred with a median follow up time of 11 months (range 1-166 months) (Table 2). CONCLUSIONS: These preliminary data suggest that primary RPLND for clinically node positive pure seminoma is a safe and efficacious treatment option that may avoid or reduce exposure to chemo or radiotherapy.
