Abstract: INTRODUCTION AND OBJECTIVE:The available data on long term outcomes of surgically treated renal oncocytoma (RO) are scant and largely outdated.Herein we describe our institutional outcomes of surgically treated RO.METHODS: We included patients who underwent surgical resection of a renal mass with pathologic diagnosis of renal oncocytoma. All cases underwent genitourinary pathology review to ascertain RO diagnosis.Disease recurrence was recorded as new radiographic evidence of a concerning renal mass warranting surveillance or intervention.Pathology from recurring disease was analyzed when available.Patient characteristics were descriptively summarized and oncologic outcomes were described using the Kaplan-Meier method.Due to paucity of events, multivariable analyses were not performed.RESULTS: A total of 561 patients underwent resection of RO between 1970 and 2012 (Table 1).Of these, 102 (18%) were mutifocal including 52 bilateral tumors at diagnosis.Clinical stage was T1 in 87% of patients.Median follow up was 10.6 years.Twenty-four patients experienced tumor recurrence at a median of 6 years.Recurring tumors were oncocytomas in 12, RCC in 2 and unknown in 10.Overall, estimated recurrence-free survival was 98, 96 and 94% at 5, 10 and 20 years, respectively. For patients presenting with multifocal disease, recurrence-free survival was 95, 87 and 87% at 5, 10 and 20 years, respectively.Multifocal disease at presentation was associated with tumor recurrence (HR[3.92,95%CI 1.75-8.75,p<0.001).Estimated recurrence-free survival is reported in Figure 1.CONCLUSIONS: While RO has excellent oncologic outcomes, a subgroup of patients presenting with multifocal disease are at significant risk of recurrence. These data support revisiting guidelines for follow up for renal oncocytoma after surgical resection.
