18F-FDG PET/CT Predicts Response to HER2-directed Neoadjuvant Therapy

Type: article
Abstract: H ER2-directed pertuzumab and trastuzumab (PT) dual therapy in conjunction with chemotherapy is becoming a standard of care for patients with high-risk HER2positive breast cancer.Recent studies suggest that a subset of these patients may be adequately treated with HER2directed therapy alone without chemotherapy.Predictive biomarkers are needed to prospectively identify patients with HER2-positive disease who may or may not be safely de-escalated from chemotherapy.Early assessment of response to neoadjuvant therapy with noninvasive imaging may facilitate tailoring of treatment regimens.Previous studies have shown that metabolic changes at 18 F-FDG PET/CT occur as early as 2 weeks after initiation of neoadjuvant HER2directed therapies.Connolly et al conducted a singlearm, multicenter phase II study (TBCR026 trial) to test the hypothesis that early metabolic changes at 18 F-FDG PET/CT could predict pCR in patients receiving neoadjuvant PT without chemotherapy. They enrolled 88 women with stage II or III estrogen receptor-negative, HER2-positive breast cancers who received four cycles of neoadjuvant PT dual therapy.Patients underwent 18 F-FDG PET/CT at baseline and after the first 15 days
