Abstract: D-Penicillamine is widely used in the treatment of rheumatoid arthritis (RA).the pharmacological properties and the therapeutic use of the drug are thoroughly discussed in this comprehensive vol ume.No doubt, D-penicillamine has powerful effects on different immunological functions and parameters, such as autoantibodies, rheumatoid factors, and immune complexes. Its use is hampered by serious side-effects, such as nephropathy and myasthenia gravislikew syndrome. It is not clear if the therapeutic value of the drug outweighs the risk of serious side-effects.A thorough controlled multicentre trial comparing D-penicillamine 125 and 500 mg per day with placebo under 36 weeks in patients with active RA (J.E. Ward)gave 'minimal changes' regarding improvement in the num ber of tender joints, grip strength and other clinical signs in patients
