Abstract: We greatly appreciate the letter by Dr Javier-DesLoges and colleagues (1) in response to our article. Their letter makes a compelling argument for the use of novel biomarker tests to detect prostate cancer in transgender women. However, we would like to clarify that we did not assume that “transgender, natal male” (transgender women) patients underwent gender affirmation surgery involving bilateral orchiectomy as their letter states. Instead, we assumed the group of transgender women who developed prostate cancer in this analysis included a combination of individuals who did and did not undergo orchiectomy. Existing evidence suggests that the risk of prostate cancer is the same in transwomen with exposure to androgen deprivation therapy who have and have not undergone orchiectomy (2), though the impact of surgery on survival following a diagnosis of prostate cancer is unknown. Having more accurate data on gender identity and gender-affirming care (including surgeries and hormone use) would help to characterize cancer risk and outcomes among transgender individuals.
