Abstract: The COVID-19 pandemic transmission process was favored by the network of urban hierarchies. The first cases were confirmed in the largest Brazilian cities and, later, in the smaller ones. Therefore, the internalization and acceleration of transmission occurred through the connections and flows of people and goods. Thus, the present research discussed how the virus entered Maranhão territory and understand the process of expansion of COVID-19 contamination by the state, from a spatial analysis. Secondary data were used from confirmed cases made available by the State Department of Health of Maranhão. For the preparation of the coropletic maps, the cartographic basis obtained from the Maranhense Institute of Socioeconomic and Cartographic Studies of Maranhão and IBGE was used. The data were worked using the QGIS 3.12.1 software. Initially, three entry points of the confirmed cases were identified in the state, São Luís, Imperatriz and Timon; subsequently, there was internalization throughout the state. Structural factors such as urban hierarchy and road work facilitated the spread of the virus and the contamination of more people because there was a similar distribution pattern between the two.
