Abstract: To the Editor: We thank Sood et al for their kind evaluation of our article, “Clopidogrel and Proton Pump Inhibitors: Safe or Not?”1 After reviewing the data presented in your letter, the following observations can be made regarding your study. It is retrospective and observational in nature and has all the limitations of retrospective studies2, 3; however, the results are consistent with the retrospective studies mentioned in our article. The sample size used is relatively small, which may predispose it to a type 1 error. The distribution of comorbidities is unknown. We need a multivariate analysis to assess the effect of proton pump inhibitors (PPIs) on coronary events as an independent predictor. The rate of protection from gastrointestinal bleeding in patients on PPIs compared to those not on PPIs suggest 0 events as compared to 4 events, thereby demonstrating the protective ability and need of PPIs in carefully selected patients. Please note that we have also emphasized in our article that the decision to start PPIs should be a carefully determined one rather than as a routine prophylaxis. We strongly agree that there is a need for a large, randomized, parallel, prospective, multicenter study to determine the effects of PPIs on antiplatelet therapy.
