Prognostic Risk Assessment and Prediction of Radiotherapy Benefit for Women with Ductal Carcinoma In Situ (DCIS) of the Breast, in a Randomized Clinical Trial (SweDCIS)

Abstract: Background: Risk assessment and prediction of radiotherapy (RT) benefit after breast conserving surgery (BCS) for DCIS are crucial. The study aim was to validate DCISionRT ® in a large randomized cohort (SweDCIS) with long term followup. Methods: Women were randomised to RT or control after BCS, between 1988 and 2000. Tumour blocks were collected and slides were sent to PreludeDx, and DCISionRT test results were reported. Associations between the continuous decision score (DS) and 10–year ipsilateral recurrences, comprising invasive breast events (InvBE) and total (DCIS+InvBE) breast events (TotBE), in women treated without RT, and RT effect on rates within Low (DS≤3) and Elevated Risk (DS>3) groups were assessed independently in Sweden and verified. Findings: In 504 women with complete biosignature data and negative margins, DCISionRT divided women into Elevated (52%) and Low Risk (48%) groups. In the Elevated Risk group, RT decreased 10-year relative TotBE and InvBE rates, HR 0·32 (0·17–0·58) and HR 0·24 (0·08–0·74), with corresponding absolute decreases in TotBE and InvBE rates of 15·5% (95%CI 5·9–25·0%) and 9·3%(2·0–16·5%). In the Low Risk group, the relative RT benefit was lower: HR 0·53 (0·28–1·02) and 0·84 (0·30–2·31), with absolute differences with RT of 5·7%(-0·8–12·2%) and 1·2%(-5·7–8·2%). Recurrences at 10-years without RT, increased with increasing DS, HRs per 5 DS-units for TotBE and InvBE were 1·5(1·1–2·3) and 1·5(0·7–2·8). Interpretation: Our data support that DCISionRT showed prognostic and predictive value, identifying women with elevated recurrence risk who benefited substantially from RT after BCS. In addition, the test identified women with low absolute risk, and minor absolute risk differences from RT. Absolute 10-year recurrence risks in the DS Low and Elevated Risk groups with and without RT are sufficiently different to indicate that DCISionRT can inform selection of individual adjuvant DCIS treatment strategy.
