Advanced Particle Therapy Delivery

Abstract: Delivery of proton therapy matured over recent years from single scattering and double scattering into intensity-modulated proton therapy pencil beam scanning (IMPT-PBS), which has been used in clinics more often. In order to enhance efficiency, conformity, and clinical use, more concepts are introduced such as line scanning, proton arc therapy, flash therapy, linear energy transfer (LET)/RBE optimization, proton imaging, and prompt gamma. The chapter covers IMPT and arc therapy in detail follow by discussing other new concepts briefly. Given the finite penetration range of proton particles and limited scatter, proton therapy delivers superior dose distributions including dose conformity, homogeneity, and sparing of the adjacent healthy tissues. Compared with traditional passive scatter proton therapy, spot-scanning beam IMPT not only eliminates the use of range modulation wheel, beam-specific block and compensator, but also has the advantages to produce a more conformal dose distribution.
