Does Mechanical Circulatory Support–Assisted Percutaneous Coronary Intervention Improve Left Ventricular Function?

Year: 2022
Type: editorial
Abstract: Ischemic cardiomyopathy (ICM) remains a leading cause of mortality and morbidity despite advances in guideline-directed medical therapy (GDMT) and defibrillator and/or cardiac resynchronization therapy (CRT) as well as other advanced therapies. Concerningly, a recent report demonstrated that <40% of patients with systolic dysfunction and new-onset heart failure will even undergo coronary artery disease (CAD) testing.1 In addition to administration of GDMT, for patients with ICM with multivessel or left main CAD and left ventricular ejection fraction (LVEF) of ≤35%, evaluation for coronary artery bypass graft (CABG) is a class I recommendation based on the Surgical Treatment for Ischemic Heart Failure Extension Study (STICHES)2; however, for some patients, CABG may be prohibitively high risk or may not be an acceptable option.
