Abstract: The discovery of artificial radioactivity triggered the birth of nuclear medicine: a peculiar medical specialty able to depict pathophysiological processes in the human body by using radiopharmaceuticals. The major boost to nuclear medicine in patient management was the development and commercial availability of hybrid positron emission tomography (PET)/computed tomography CT and single photon emission tomography (SPECT)/CT scanners. The most important role of PET/CT with radiolabeled choline is restaging of prostate cancer after radical prostatectomy or radiotherapy. The early assessment of response to therapy by interim PET/CT in patients with lymphoma can predict remission of disease and complete response, with high positive predictive value. Common guidelines suggest the use of scintigraphy with labeled leukocytes in clinical and laboratory suspicion of osteomyelitis and spondylodiscitis, infection of joint prosthesis, fever of unknown origin, infective endocarditis, and vascular graft infections. Radioguided surgery enables surgeons to identify lesions or tissues that have been preoperatively marked with radioactive substances.
