Abstract: Intro: The US Neisseria meningitidis nongroupable urethritis clade (US_NmNG UC) has caused multi-outbreak in the US since 2013 and then being reported immigrating to the UK in 2019. Aims of this work to describe a cluster of 19 US NmNG UC isolates collected from urethritis cases of young adult men in Vietnam in the years 2019 and 2020. Methods: The isolates were confirmed as Nm with sodC rt-PCR and API-NH, identified one of six possible serogroups using both rt-PCR and latex agglutination, and determined antimicrobial resistance (AMR) with E-test for Penicillin, Cefotaxime, Meropenem, Azithromycin, Ciprofloxacin, and Rifampicin according to the CLSI. Isolates then underwent Whole-Genome Sequencing to describe molecular and phylogenetic characteristics. Findings: All 19 isolates identified as US_NmNG UC, being typically genetype NG:P1.5-1,10-8, FetA 3-6;ST11/CC and unique FHbp ID 896. All were reduced susceptible to penicillin (MIC at 0.125 – 0.38 mg/mL), nine of them resisted to ciprofloxacin (MIC at 0.19 – 3 μg/mL) but susceptible to the others. A network based on gene-by-gene comparation that was displayed by Splitstree showed isolates in Vietnam, in the UK and in New York generating a monophyletic clade, deriving from the clade of Ohio. Discussion: While other isolates of the US NmNG UC are sensitive to ciprofloxacin, those in Vietnam show highly resisted to the antibiotic thanks to bearing new gyrA allele that has multi-mutation of T91I and D95A and S87R of parC. The mutated fragment is likely original from gonococcal donors through recombination between two species, relying on a further analysis with the RDP4. Conclusion: The study provides another evidence of the spread of the US NmUG UC outside the border of the US beside one in the UK. This may raise a concern about the emerging and widespread antibiotic resistance with a high rate of ciprofloxacin among urethral Nm isolates found from this cluster.
