OP30.02: Endometrial echo thickness in postmenopausal women: are measurements on sonohysterography compatible with measurements on transvaginal sonography?

Year: 2016
Type: conference-abstract
Abstract: To compare measurements of the endometrial echo (EE) thickness obtained by transvaginal sonography (TVS) with the sum of the endometrial layers measured at time of saline infusion sonohysterography (SIS). A prospective study of women 50 years and older who underwent endometrial evaluation with both TVS and SIS. Patients' characteristics and sonographic findings were assessed. EE was measured according to recommendations of the International Endometrial Tumor Analysis (IETA) group. Patients with poor visualisation of EE or intracavitary fluid on TVS were excluded. The double layer of EE on TVS was compared to the sum of the two endometrial layers measured on SIS. EE calculations between the two imaging modalities were compared after excluding cases with intracavitary lesions. TVS and SIS were completed in 371 patients. The median age was 59 years (range 50–85) and mean BMI was 34.0 ± 17.9 kg/m2. The main indication for a scan was postmenopausal bleeding (71.1%). Excluded patients included those with intracavitary fluid, 34/371 (9.2%), and poorly visualised EE, 53/371 (14.3%). In the remaining 284 women, the mean double layer EE thickness on TVS was 9.8 ± 2.6 mm versus 3.6 ± 2.6 mm (p < 0.0001) on SIS. This significant difference remained after exclusion of 145 women with a finding of a polyp on SIS (n = 139, 7.5 ± 4.4 mm on TVS vs 4.0 ± 2.6 mm on SIS, p < 0.0001) and focal thickening (n = 83, 6.6 ± 4.2 mm on TVS vs 3.6 ± 2.1 mm on SIS, p < 0.0001). Exclusion of 32 cases with submucosal fibroids as the only finding on SIS did not impact the results (n = 51, 6.7 ± 4.6 mm on TVS vs 3.6 ± 2.3 mm on SIS, p = 0.001). The sum of the endometrial layers calculated at time of SIS is significantly less than the EE thickness measured on TVS. The difference remains significant even after excluding cases with polyps, focal thickening and submucosal fibroids. The findings in this study suggest that the definition of thin endometrium used in TVS cannot be applied to SIS.
